How much will my copayment be?
Your copayment is a fixed out-of-pocket amount paid by an insured (you) for covered services. This amount will vary according to your individual insurance policy. No worries! Community Family Care will find out this amount prior to you arriving at your appointment, but please be aware that there may be a copay due at time of service.
I don’t have my insurance card with me, can I still receive treatment?
Your insurance information will be collected at time of scheduling appointment, so if your insurance card is forgotten or misplaced at time of appointment, it won't be a problem!

What is your new patient procedure?
At your New Patient appointment, you will be asked about your health concerns and goals. An in-depth medication review will be done, if applicable. A thorough medical history and physical examination will be obtained, with an individualized treatment plan being formulated from your exam findings and according to your preferences.
